The everlasting question in chronic inflammatory dermatoses is what comes first: the immunodysregulation of T lymphocytes or the epithelial changes. The answer to this hen and egg question swings back and forth. T lymphocytes were in the focus as culprits of the diseases during the ciclosporin era: the observation that the T-cell-directed drug ciclosporin exerted therapeutic effects stimulated research into the role of lymphocytes in the pathophysiology of chronic inflammatory skin diseases.
