A prospective study in 82 atopic dermatitis (AD) patients and 20 healthy controls investigated whether Candida albicans colonization was related to the severity of AD and whether antifungal treatment modified various disease markers. A high incidence of C. albicans isolates were detected in severe (40%) and mild-moderate (36.7%) AD. There was a significant increase in C. albicans specific IgE that could be correlated with the SCORing Atopic Dermatitis (SCORAD) score (p<0.001). After fluconazole treatment there was a mild-moderate significant improvement in the SCORAD, a decrease of eosinophilic cationic protein (ECP), intercellular adhesion molecule-1 (ICAM-1) and a significant decrease in C. albicans specific IgE.
